Introduction

Referral Management (Carelink) is a Totalmobile cloud-hosted application that enables the management of care at home and reablement services. The care plans and service users’ preferences defined in Referral Management are then used as scheduling criteria. Along Dynamic Scheduling (Optimise) and Mobile Working (Mobilise), Referral Management (Carelink) offers a solution to help care providers to deliver effective and efficient care.

The following diagram shows the integration between Totalmobile products in a typical care context such as the delivery of homecare and reablement services.

  1. Social Care System: This is a third-party system where service user details are recorded.
    Referral Management provides a set of REST APIs to help customers build their own integration with the social care system. There is an option to use Totalmobile Integration but a custom integration with a third-party system will be required.

  2. Referral Management (Carelink): Totalmobile application which records service user details, care plans visits, one-off visits, service user preferences and breaks during the service. Service user details can be loaded using the REST APIs as part of the integration or they can be recorded manually. Referral Management dispatches visits to Dynamic Scheduling based on service user care plans. Also, Referral Management uses Optimise Scheduling data to provide a feedback of the capacity available for a new request in the service.

  3. Dynamic Scheduling (Optimise): Totalmobile application which manages resources and rosters, and dynamically schedules visits. Dynamic Scheduling receives care plan visits dispatched from Referral Management with all relevant data recorded to deliver the care and then schedules them based on the resources available and any scheduled criteria defined.

  4. Mobile Working (Mobilise): Totalmobile mobile application which provides care workers with all relevant information to carry out their daily visits and enable them to send updates to the back office in real time via a mobile device. The Mobile Working app receives care plan visits dispatched from Dynamic Scheduling after they have been scheduled and allocated to the care workers.
