Form Sections and Controls

Forms are made up of elements. An element can either be a section, which groups together other elements and separates information on your forms where needed, or a control, which fulfils a specific function within the form. These sections and controls help you build simple and complex forms depending on your requirements.

Form Control Fields

The following fields are common across all or most form controls:

Field Name

Description

Reference

A unique reference for the control within the form. If you want to use dynamic text as described below, or create links between controls, for example to source a list in the List Element Control based on the answer from a previous element, you select the control reference.

The reference is automatically defined, but you can edit it if required.

Text

Enter a mandatory description for the control. This description appears in the form. You can change the text formatting and colour.

Dynamic Text

Enter any text, such as a question, that includes references to other elements in the form. Select the reference from the Insert Element dropdown as part of the text.

Folios

Select Edit Folios to map a folio to the control.

In the Folios dialog, select Folio, Element or URL from the dropdown. Add the appropriate content and select Add , then select OK to confirm.

Image

Select Select Image to add a contextual image for the control. For more details refer to Image Context for Form Controls.

Promoted

Toggle to On to promote the control when the form has a status of In Progress, Complete or Submitted. Promoting means that when you return to the details screen of the visit, the value of the promoted field displays under the form task.

If required, select the Promoted Prompt field and enter a heading for your promoted answer.

Mandatory

Toggle to On to make the control mandatory within the form.

Hidden

Toggle to On to prevent the control and any associated information from appearing within the form on the user's device. For example, you might select this option in the Score control if the user does not need to see the relevant score calculations.

Read-only

Select an option from the dropdown to define whether the control is read-only. You can select After Map In to make the control read-only after it has been pre-populated, for example to ensure the result of a search query is not editable.

Enrich Content

Toggle to On to enable users to use rich text formatting, for example bullet points, when entering data. The formatting is maintained in subsequent forms and reports.
